MR 021 - ABC FIRE EXTINGUISHER - A properly tested and maintained ABC type fire extinguisher of adequate size shall be provided in the machine rooms, control rooms, and machinery spaces and control spaces per ASME A17.1-8.6.1.6.5. The fire extinguisher in the machine rooms, control rooms, and machinery spaces and control spaces is to be of sufficient size to allow workers within the room to exit safely in the case of a fire within the machine rooms / control rooms occurs during their maintenance procedures. The extinguisher is not meant for usage for returning to the room to fight the fire. The extinguisher is to be located in an area of the room that will allow easy access to the extinguisher by workers. It is recommended that when possible, the extinguisher be located near the jamb side of the elevator room entrance door. ( last checked Apr 2022 ) (7/10/24) ( 3/31/25 )

M 006 - BEING REPAIRED - This unit was not in operating condition at the time of the inspection. This unit is required to be inspected once the unit is returned to operational status. The elevator device is subject to continued invoices from our office unless the elevator device is properly removed from service and properly sealed. R 001 - FIRE FIGHTERS EMERGENCY OPERATION LOG - All elevators provided with Firefighters Emergency Operation shall be subjected monthly, by authorized personnel, to Phase I recall by use of the key switch, and a minimum of one-floor operation on Phase II. Deficiencies shall be corrected. A record of findings shall be available to elevator personnel and the authority having jurisdiction per ASME A17.1-8.6.11.1.(*6-4-21*)(*3-3-22- violation not corrected.*) ( 3/31/25 )
T 001 - SAFETY TEST - Shall perform annual required safety tests as required per ASME A17.1-8.11 or A18.1 -10. The completed form shall be left on-site within the Maintenance Control Program (MCP) or in a location readily available to the inspector. The safety test form will be reviewed during the next on-site state inspection.
EC 003 - EMERGENCY LIGHTING - The in-car emergency car lighting shall be operational under normal power failure conditions per ASME A17.1-2.14.7.1.3. ( 7/10/24 ) ( 3/31/25 )
