AD 000 - ADJP AND VAR1 FAILED INSPECTION NO ACCESS ---This elevator device is currently under an adjudication (legal) process for failure to comply with previous inspections. The elevator is subject to invoicing of inspection attempts and certificate renewal fees until such time the elevator is in compliance, properly lowered, and sealed or removed from the building. Notify our office once the unit is in compliance. No elevator may operate unless a certificate of operation is obtained. AD 007- REINSPECT REQUIRED - THIS LIFTING DEVICE SHALL NOT OPERATE UNTIL ALL VIOLATIONS ARE CORRECTED AND A PASSING RE-INSPECTION OCCURS. When this notice is returned advising that violations are corrected, an inspector will be dispatched to perform a re-inspection. If there is a need to have the re-inspection scheduled for a specific date and/or time, please indicate this at the time the notice is returned.
AD 008 - UNABLE TO ACCESS - INSPECTION OF THE LIFTING DEVICE LISTED IN THIS LETTER IS REQUIRED. AN INSPECTOR FOR THE STATE OF OHIO RECENTLY ATTEMPTED THIS REQUIRED INSPECTION AND WAS UNABLE TO GAIN ACCESS TO THE DEVICE. M 001 - REMOVED FROM SERVICE - This elevator device is removed from service due to violations that do not provide for reasonable safety and shall not operate per the Ohio Revised Code section 4105.
M 010 - NOT PROPERLY LOWERED - The unit has had the fuses removed, but the doors may not be secured and the car and/or counterweights may not be properly lowered to the pit area. The unit may or may not return to service at sometime in the future. A seal and/or padlock has been placed on the mainline electrical disconnect giving notice that the unit may not be operated without contacting the Chief Elevator Inspector. The elevator device is subject to continued invoices from our office unless the elevator device is properly removed from service and properly sealed. Note: A properly lowered unit means; the fuses removed, power from the mainline to controller disconnected, all hoistway doors permanently secured. Traction elevator cars and counterweights shall be lowered to the pit and cables removed. Hydraulic elevators shall have a section of pipe removed. M 004 - ALTERATION PERFORMED - PERMIT REQUIRED - An alteration permit shall be obtained due to alterations having been performed on the equipment. M 005 - ALTERATION PERFORMED - Alterations shall conform to the applicable requirements and tests of ASME A17.3 per ASME A17.1 and ASME A17.1-8.7.1.1.
